2025 grant opportunity
Grants for 2025 will be awarded to organizations addressing one or a combination of the following health needs priorities. When filling out the form, please check which health needs your organization addresses. If your organization does not address one of these health needs, please note that it will not be in consideration for this grant.
- Mental and behavioral health
- Home insecurity
- Substance abuse and prevention
These priorities have been identified as opportunity areas in the community and are part of the 2022 Greater Lowell Community Health Needs Assessment conducted on behalf of Lowell General Hospital and the Greater Lowell Health Alliance.
Awarded grants cannot be used to fund capital or overhead expenses. Non-profit organizations and public entities (such as municipalities, schools, health institutions and services) are eligible to apply. Priority will be given to agencies whose service area is in the communities of Billerica, Chelmsford, Dracut, Dunstable, Lowell, Tewksbury, Tyngsboro or Westford. Preference will be given to projects that serve low-income, vulnerable or at-risk communities. Collaborative efforts are also encouraged.
Award criteria
Completed applications will be reviewed on the following criteria:
- Well-designed project/program that fits into the priority area(s) with clear, measurable goals and objectives
- Clear explanation of the project/program and demonstration of the effect it will have on increasing services provided to target population(s)
- Clearly identified and demonstrated health needs supported by available data
- Realistic timeline for evaluation and implementation of project/program
- Explanation of expected outcomes and a demonstrated plan for evaluation to measure project/program success
- Demonstrated collaborative efforts with other community organizations that are part of the system of care for the stated target population/s
- Plan for sustainability of funded project/program in future years (if applicable)
- Detailed project budget
Application process
The 2025 application process is fully online. To ensure your application is considered, it must be submitted through the form below. Each organization may submit only one proposal as the lead organization but can be listed as a collaborator on additional proposals. Please include the following for your application to be accepted:
- Updated list of your Board of Directors
- Federal tax-exempt letter including tax identification number
- Most recent 990 filing
- Current W-9
Deadlines
All grant applications must be received no later than 5 p.m. on Wednesday, December 20, 2024. No extensions or exceptions will be allowed on the application deadlines.

Successful applicants will be expected to:
- Submit a summary report 6 months after receiving the award and within 3 months following the completion of the funded project.
- Participate in a video project for the 2025 Lowell General Hospital Ball for Community Health Initiatives.
- Send two representatives to the Ball at the InterContinental Boston on Saturday, March 1, 2025, at 6 p.m. Ticket is complimentary, and attendance is optional.
