Tufts Medicine Care at Home Earns Prestigious Heart Failure Certification

Tufts Medicine is proud to announce that Care at Home has earned the American Heart Association’s Home Health Heart Failure certification, the first home health organization in both Massachusetts and New Hampshire to receive this certification.

The certification represents a significant step forward in how heart failure care is delivered in the home setting, and recognizes our commitment to high-quality, evidence-based care for patients with heart failure.

This certification is based on rigorous standards that evaluate home health agencies on a number of important criteria, including program management, patient and caregiver education, care coordination, clinical management and ongoing quality improvement. Meeting these standards is a testament to the strength of our care and the dedication of our team.
