  • Chronic facial pain: Usually starts in the jaw muscles, such as with TMJ, but can also be caused by nerve or sinus conditions
  • Chronic migraine: Headaches occur daily or nearly every day, with severe migraines at least once a week
  • Cluster headache: A rare but intensely painful type of headache with attacks that can occur several times per day for weeks at a time
  • Episodic migraine: Severe migraine headaches happen at least two or three times per month
  • Tension headache: Often caused by muscle tightness, stress, or posture issues
  • Secondary headaches: Caused by underlying conditions such as sinus problems, high blood pressure, or medication overuse

Testing

Figuring out what’s causing your migraines or facial pain can take a little time, but it’s worth it to get the right treatment. During your first visit, your doctor will ask about your symptoms, family and medical history, and any patterns or triggers you’ve noticed.

Some of the tests we may use include:

  • Blood tests: Look for underlying issues that could contribute to headaches or facial pain
  • CT scans: Take detailed images of your brain and skull to look for structural problems
  • Eye exams: Evaluate vision problems that could trigger headaches
  • MRI: Uses magnets to create detailed pictures of the brain and surrounding tissues
  • Neurological exams: Check how your nerves and brain are working
  • Sinus X-rays: Check for sinus issues that may be causing facial pain
  • Urine tests: Help identify medical conditions that may play a role in your symptoms

These tests help your care team understand what’s happening and create a treatment plan tailored to you, so you can find relief and get back to your daily life.

Treatments

Once we know the type of headache or facial pain you have, we focus on relieving your symptoms and addressing the underlying causes. Our goal is to help you feel better, prevent future episodes, and get back to the activities you enjoy without pain interfering with your life.

  • Chronic facial pain: If tight or overworked facial muscles are causing pain, we often recommend a custom mouth guard paired with physical therapy to relax the muscles. In more complex cases, we work closely with orofacial pain specialists at Tufts University School of Dental Medicine for advanced evaluation and care.
  • Chronic migraines: Treatment usually combines medications with physical therapy to relax tight muscles in the neck and shoulders. In some cases, long-acting muscle relaxants like Botox can be used to prevent headaches before they start, helping reduce both frequency and severity.
  • Cluster headaches: These headaches are extremely painful and can come on suddenly, so we treat them aggressively. Immediate relief may include migraine medications or oxygen therapy, while preventive treatments like calcium-channel blockers can help stop attacks from returning.
  • Episodic migraines: Our first priority is to stop the headache quickly and safely. Once the pain is relieved, we may recommend medications that prevent future migraines and help reduce how often they occur, so you can stay ahead of your symptoms.
